Complete Care Walk & Inspire Campaign 2025, $100 Retail Gift Card Sweepstakes (HIPAA Compliant)

  1. Eligibility: The sweepstakes is open to individuals who are 18 years or older at the time of entry. Employees, contractors, agents, affiliates, and immediate family members (spouse, parents, children, siblings) of employees of Complete Care (“Sponsor”) are not eligible to participate. Participants must already be patients of Complete Care and must be receiving services from Complete Care at the time of entry to be eligible to participate. Participation is voluntary.
  2. Sweepstakes Period: The sweepstakes begins on March 1, 2025, at 12:00 AM EST and ends on March 27, 2025, at 3:59 PM EST (the “Sweepstakes Period”). Entries received after the Sweepstakes Period will not be eligible for entry.
  3. How to Enter:

Registration & Pledge:

All participants (patients) must register for the sweepstakes by completing the registration form at the clinic front desk. During registration, participants must pledge the number of miles they commit to walking between March 1, 2025, and March 27, 2025.

Please check with your doctor before registering and engaging in this physical activity.

Tracking and Celebrating Miles: Clinics will track miles walked by patients using a visual “Star Tracker” displayed on clinic whiteboards. To verify miles, participants must show proof of miles walked via a fitness app of their choice. Each verified mile walked will earn one star on the board and count as one (1) entry into the sweepstakes.

Earning Entries:

  • Patients: Each proven mile walked during the sweepstakes period earns one (1) entry.

Clinic Team Tracking:

  • The clinic with the highest total miles walked (most stars) will receive a team-building prize valued at $30 per person.

Raffle Entries & Drawing:

  • Each verified mile walked, as tracked by the Star Tracker, will count as one (1) entry into the raffle for the $100 retail gift card prize. The more miles walked, the greater the chances of winning. At the end of the Sweepstakes Period, a random drawing will be held to select two winners from the pool of eligible entries.
  • Prize: Each of the two (2) winners will receive a $100 retail gift card (the “Prize”). The Prize is non-transferable, non-exchangeable, and has no cash value. The Sponsor reserves the right to substitute the Prize with one of equal or greater value, at its sole discretion. Any taxes or fees associated with the Prize, including but not limited to federal, state, or local taxes, are the sole responsibility of the winner.

5. Winner Selection and Notification:

The winner will be selected by a random drawing from all eligible entries received during the Sweepstakes Period. The drawing will take place on April 4, 2025, and the winner will be notified by phone within three (3) business days of the drawing. The winner must respond to the notification within forty-eight (48) hours to claim the Prize. If the winner fails to respond within the specified time frame, an alternate winner may be selected.

6. Disqualification and Final Decision:

The Sponsor has the exclusive right to determine the winner. All decisions made by the Sponsor, including winner selection, are final and non-disputable. If a selected winner does not meet eligibility requirements, fails to respond within the specified time, or otherwise violates these terms, the Sponsor reserves the right to disqualify the individual and select an alternate winner.

7. Privacy and Compliance with HIPAA:

The Sponsor is committed to protecting participants’ privacy and compliance with the Health Insurance Portability and Accountability Act (HIPAA). By entering, participants consent to the collection of non-medical personal information (name, email, phone number) solely for the purpose of administering the sweepstakes and notifying the winner. No medical or treatment information will be used or collected for this sweepstakes. Personal health information (PHI) will not be requested or included in any entry form.

Any personal information collected will be used exclusively for the sweepstakes and will not be shared with third parties, except as required by law. Participants have the right to request removal of their personal information at any time prior to the drawing.

8. Publicity:

By entering, participants consent to the use of their name, image and likeness for promotional purposes, without further compensation, unless prohibited by law. No medical or health-related information will be publicly disclosed.

9. Liability Release:

By entering, participants agree to release and hold harmless Complete Care, its affiliates, officers, directors, employees, agents, contractors, and their respective successors, heirs, and assigns (collectively, the “Released Parties”) from any liability, loss, damage, injury, or claim of any kind arising from participation in the sweepstakes, the acceptance, possession, use, or misuse of the Prize, or any other matter related to the sweepstakes.

PARTICIPANTS AGREE THEY PARTICIPATE ENTIRELY AT THEIR OWN RISK WHILE AWARE OF THE RISKS ASSOCIATED WITH THIS ACTIVITY, WHICH MAY INCLUDE, BUT ARE NOT LIMITED TO, PHYSICAL OR PSYCHOLOGICAL INJURY, PAIN, SUFFERING, ILLNESS, DISFIGUREMENT, TEMPORARY OR PERMANENT DISABILITY (INCLUDING PARALYSIS), ECONOMIC OR EMOTIONAL LOSS, AND DEATH. PARTICIPANTS UNDERSTAND THAT THESE INJURIES OR OUTCOMES MAY ARISE FROM THEIR OWN OR OTHERS’ NEGLIGENCE, CONDITIONS RELATED TO GATHERING IN LARGE CROWS, TRAVELING TO OR FROM THE EVENT, PHYSICAL EXERTION FROM WALKING, OR THE CONDITION OF THE ACTIVITY LOCATION(S). NONETHELESS, PARTICIPANTS ASSUME ALL RELATED RISKS, BOTH KNOWN OR UNKNOWN TO THEM, OF THEIR PARTICIPANTION, INCLUDING TRAVEL TO, FROM AND DURING THIS ACTIVITY. PARTICIPANTS ACKNOWLEDGE THAT ENGAGING IN THIS ACTIVITY MAY CARRY WITH IT THE POTENTIAL FOR DEATH, SERIOUS INJURY, AND PROPERTY LOSS AND AGREE TO HOLD HARMLESS COMPLETE CARE AGAINST ANY AND ALL CLAIMS, SUITS OR ACTIONS OF ANY KIND WHATSOEVER FOR LIABILITY, DAMAGES, COMPENSATION OR OTHERWISE BROUGHT BY PARTICIPANTS OR ANYONE ON THEIR BEHALF, INCLUDING ATTORNEY’S FEES AND ANY RELATED COSTS, IF LITIGATION ARISES PURSUANT TO ANY CLAIMS MADE BY PARTICIPANTS OR BY ANYONE ELSE ACTING ON THEIR BEHALF RELATING TO THEIR DECISION TO PARTICIPATE.

10. Force Majeure:

In the event of circumstances beyond the Sponsor’s control, including but not limited to natural disasters, technical malfunctions, or legal regulations, that may affect the execution of the sweepstakes, the Sponsor reserves the right to modify, cancel, or suspend the sweepstakes at its sole discretion.

11. Governing Law:

This sweepstakes is governed by the laws of the State of Florida, without regard to its conflict of law principles. Any disputes or legal proceedings related to the sweepstakes shall be subject to the exclusive jurisdiction of the state or federal courts located in Maitland, Florida.

12. General Conditions:

By entering, participants agree to comply with these Official Rules and the decisions of the Sponsor, which are final and binding. Any violation of these Official Rules or failure to meet eligibility requirements as determined by the Sponsor in its sole and absolute discretion will result in disqualification. The Sponsor reserves the right to disqualify any participant it believes to be tampering with the entry process or violating the spirit of the sweepstakes.

13.Sponsor:

This sweepstakes is sponsored by Complete Care Centers, LLC which is located at 555 Winderley Place, Maitland, Florida 32751.

By participating in this sweepstakes, participants acknowledge that they have read, understood, and agree to be bound by these Official Terms and Conditions.
